OCF-1: Accident Benefits Application Package

What is the Accident Benefits Application Package (OCF-1)?

According to the Financial Services Regulatory Authority (FSRA) of Ontario, you must complete a series of forms if you are seeking compensation for your injuries. One of them is the OCF-1 (Accident Benefits Application Package). If you were injured in an automobile accident on or after November 1, 1996, your auto insurance provider must complete and submit the OCF-1, also known as the Accident Benefits Application Package. In addition, your insurer may request that you enclose with this application additional forms, such as the OCF-2, OCF-3, OCF-5 and/or OCF-23. Submit the OCF-5 only after you have spoken to a lawyer.

In addition to requiring you to submit this package, your insurer may ask that you enclose with this application additional forms such as the OCF-2, OCF-3, OCF-5 and/or the OCF-23. Upon reviewing your application package, the insurance company will contact you to discuss your potential benefits.

Submitting this package will make it easier to establish whether you qualify for benefits. However, you must complete it correctly so that your insurance provider compensates you fully and punctually. We, therefore, recommend you hire a personal injury lawyer so that they can help you complete the form accurately to ensure that you receive the compensation and benefits you rightfully deserve.

     The OCF-1 asks you for information on:

    • your accident,
    • your auto insurance policy,
    • your employment status,
    • your health,
    • what your income replacement determination is;
    • whether you are a student or a family caregiver; and
    • other insurance and collateral payment information.

    Insurance companies scrutinize claimants’ application packages, so ensure to complete your application package as accurately as possible to receive all the benefits to which you are entitled. We recommend you contact an experienced personal injury lawyer who will help you complete this application package.

    Our personal injury lawyers know OCF-1 very well. Our lawyers will ensure that the OCF-1 lists your injuries. They will clarify which benefits apply in your case, explain the confusing aspects of the package, and submit the package correctly on your behalf. They will also deal with the insurance provider on your behalf. Avoid signing anything that seems unclear to you. Here again is where a personal injury lawyer can help you. We will also help you complete the paperwork accurately and meet all the filing deadlines.
