What is the Expenses Claim Form (OCF-6)?

You can complete the OCF-6, also known as the Expenses Claim Form, and submit it to your auto insurance provider so that you can claim expenses you incurred as a result of your accident. If you wish to claim expenses that your other forms will not cover, you must actively request the OCF-6.
