In addition to your personal information (name, address, phone number), the OCF-6 requires a list of goods and services you wish to claim as expenses resulting from your accident. This list will include a description of each good and/or service, including the names of service providers. In addition, you need to enclose the many bills and receipts from those goods and services or explain why you cannot provide these.

- clothing or personal belongings lost, damaged, or destroyed in the accident.
